Instructions

  1. Please complete all fields in the below application.
  2. ProfileGorilla will send you an email confirmation receipt of submission.
  3. Once your application has been submitted, you will be added to our 1099 roster at that time.
  4. When your services are required, ProfileGorilla will reach out to you to create your Profile Login and complete your remaining Profile for Custard.
  5. If any items are missing, ProfileGorilla will contact you via email or phone.
